Annette Grace Formantes entered peacefully into rest in San Mateo, California on November 1, 2023. She was 60 years old.

Born January 17, 1963 in the Philippines to parents Rosenda and Paulo Formantes, Annette worked as a health care administrator for 20 years. She is survived by many loving family members and friends and will be deeply missed.

Arrangements have been entrusted to Grissom’s Chapel & Mortuary.
